What is Back Pain?

Herniated (Bulging) Discs

Herniated discs, also known as slipped discs, are a frequent cause of back pain. This condition occurs when the soft inner gelatinous core of an intervertebral disc in the spine is pushed out through a tear or rupture in the outer fibrous ring. This can cause pain by putting pressure on the spinal nerves. Herniated discs often result from improper lifting, sudden twisting motions, excessive forward bending, or trauma from an injury or accident. The pain typically feels like a shooting, sharp ache in the back that can radiate down the legs. It may be accompanied by numbness, tingling sensations, or weakness in the legs or feet. The pain tends to worsen with bending, lifting, standing, or walking.
Degenerative disc disease (DDD) often causes chronic low back pain. It occurs when the discs between the vertebrae (spinal bones) in the spine break down from wear and tear or injury. As the discs lose integrity, they cannot cushion the vertebrae as well. This causes a dull ache in the back that may radiate into the buttocks and legs. Activities that place extra pressure on the spine — like heavy lifting, bending, twisting, or vibration from driving — can trigger or worsen pain from DDD. The discs also dry out over time and lose height, which may lead to osteoarthritis or bone spurs that compress nerves, resulting in inflammation and more back pain.
Spinal stenosis is a narrowing of the spaces within the spine, which can put pressure on the nerves in that area. This deformation is often caused by the natural aging process, arthritis, or a bulging disc. As the spaces narrow, it may cause back pain, numbness, and weakness. Spinal stenosis usually appears in the lower back and neck areas. The pain worsens when walking and improves when leaning forward or sitting. It can range from mild to severe.
This type of strain occurs when the muscles or ligaments in the back are overstretched or torn. Activities that involve sudden twisting, lifting of heavy objects, overextending, or overusing the back can cause this problem. Sports like golf, football, weightlifting, and gymnastics often put a serious strain on the back muscles and ligaments. Even simple things, such as bending over to pick up an object or turning the wrong way when getting out of a chair, can lead to a strain. There may be localized pain in a particular spot or muscle of the back or more generalized pain across the entire back. The affected area can be tender to touch. This type of pain tends to worsen with movement or activity.

Gary Yen, MD

Dr. Gary L. Yen is Board certified in Physical Medicine and Rehabilitation and has a sub-specialty in Pain Medicine. Dr. Yen grew up in Long Island, NY and received his medical degree from Ross University. He completed his internship in Internal Medicine at Lutheran Medical Center in Brooklyn, NY and performed his residency in Physical Medicine and Rehabilitation at Stony Brook University, where he was chief resident.

Dr. Yen completed a pain management fellowship at RehabNY in Buffalo, NY, during which he served as the assistant athletic trainer to the Buffalo Bison, the Minor League AAA baseball team for the Cleveland Indians, as well as the Niagara University men’s and women’s collegiate hockey team.

The former chairman of the Physical Medicine & Rehabilitation Division of the Pain Management Department at CentraState Medical Center, Dr. Yen specializes in the non-operative treatment of a variety of joint, muscular, nerve, skeletal, spine and sports related injuries. He treats acute and chronic pain syndromes and performs electrodiagnostic studies. Dr. Yen also performs variety of interventional pain procedures and minimally invasive spine surgery for the relief of pain. He utilizes conservative treatments such as physical therapy, chiropractic care and acupuncture in conjunction with other procedures to enhance long term benefits. Dr. Yen believes in working with patients to create individual treatment plans.

When not attending to his patients, Dr. Yen likes to play golf, cook, and has a passion for playing the saxophone and piano. He is the proud father of two boys.
